Job summary

Seaport Scripps Home Health in Oceanside, CA is seeking a WOC RN (CWOCN) to join our home health team. You will assess wounds, ostomies, continence issues and provide advanced wound care, education, and care planning for complex patients.

The ideal candidate has CWOCN certification, 2+ years in home health or public health, CPR, and valid RN license. You will supervise HHAs and collaborate with physicians to deliver compassionate, high-quality care.

Qualifications

Graduated from an approved school of nursing and licensed to practice.
CWOCN certification preferred for role.
Minimum of two years in public health or home care nursing is preferred.
Maintains current CPR certification.
Must be a licensed driver with reliable transportation.

Responsibilities

Conduct comprehensive wound, ostomy, and continence assessments.
Monitor, document, and manage wound healing progress and interventions.
Provide advanced wound care including debridement, dressings, and negative pressure therapy.
Educate patients and caregivers on wound, ostomy, and continence care.
Collaborate with physicians and care teams to update care plans.
Supervise home health aides and coordinate equipment/services as needed.
