New York Families for Autistic Children is seeking a part-time Community Habilitation Worker for Saturdays in Williamsburg, Brooklyn, NY. The role focuses on person-centered supports, independence, and community integration for individuals with developmental disabilities.

Qualifications

Responsibilities

  • Provide services to persons served in home and community according to their Habilitation Plan.
  • Document Habilitation services contemporaneously per requirements.
  • Monitor safety and welfare; notify supervisor of concerns.
  • Support community integration and attendance checks.
  • Attend staff meetings and complete trainings.
  • Complete required paperwork daily.
