Job summary

The role supports Veterans by guiding them through the Whole Health system, orienting them to services, and facilitating health education and goal setting. The position emphasizes group-based and individual sessions, assisting with personal health planning, and coordinating with clinical care teams to meet Veterans health goals.

Qualifications

• One year of specialized experience equivalent to GS-05 in the normal line of progression.
• Experience as group leader in community or residential recreation or training programs.
• Experience as a counselor or instructor in a program related to social services or health/well-being.

Responsibilities

• Leads Whole Health Orientation, Introduction to Whole Health, and Taking Charge of My Life and Health Groups after training.
• Provides information and advocates for Veterans' personal health goals and plans within the clinical care team.
• Navigates and orients Veterans to services, hours, locations, staff, tools, and resources in the Whole Health system.
• Facilitates group programs exploring health and well-being, connects Veterans with community supports, and supports follow-up actions.
