Job summary

Emblem Home Health & Hospice in the West Phoenix area is seeking a Licensed Physical Therapy Assistant (PTA) to provide hands-on care under a Physical Therapist, with a flexible schedule that respects your lifestyle. Deliver treatments per the plan of care, guide patients through therapeutic exercises, monitor progress, and document outcomes while collaborating with a supportive team.

Arizona licensure, CAPTE-approved PTA program, CPR, and a valid driver's license with reliable transportation

Qualifications

• Licensed Physical Therapy Assistant (PTA) in the state of Arizona.
• Graduate of a CAPTE-approved PTA program (passed national exam).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Deliver physical therapy treatments per PT and physician’s plan of care.
• Guide patients through therapeutic exercises and mobility training.
• Monitor and report patient progress and condition changes.
• Educate patients on mobility aids and prosthetic devices.
• Maintain accurate clinical notes and participate in team meetings.
