Responsibilities

• Conduct virtual wellness and health screening appointments for Medicare patients.

• Prepare patients for provider visits with complete screening data.
• Document assessment findings in the EMR system.
• Follow protocols to meet quality and compliance standards.
• Manage a daily schedule of about 12–15 patient appointments.
• Collaborate with nurses and providers to support value-based care.
• Maintain professional, compassionate patient communication.
• Ensure timely completion of all required documentation.

Compensation: $25.00 - $34.00 per hour

About the Opportunity

Addison Group is seeking a Licensed Practical Nurse (LPN) on behalf of our client for a temporary contract opportunity within a value-based care team. This is a unique, non-patient-facing role that focuses on completing virtual health screenings and annual wellness assessments for Medicare patients prior to provider visits.

This opportunity is ideal for an LPN with strong communication skills who enjoys patient interaction over the phone and is looking to gain experience within a highly respected physician organization.

Key Responsibilities

• Conduct virtual wellness and health screening appointments with Medicare patients
• Complete comprehensive health assessments using established templates and workflows
• Gather and document patient history, risk factors, and preventive care information
• Prepare patients for upcoming provider visits by ensuring required screening information is complete
• Accurately document assessment findings within the electronic medical record system
• Follow standardized protocols to ensure quality and compliance standards are met
• Manage a daily schedule of approximately 12-15 patient appointments
• Collaborate with nurses, quality teams, and providers to support value-based care initiatives
• Maintain professional and compassionate communication during all patient interactions
• Ensure accurate and timely completion of all required documentation

Requirements

• Active LPN license required (North Carolina or multistate license)
• Previous health screening, wellness visit, population health, or care management experience preferred
• Strong customer service and telephone communication skills
• Ability to build rapport with patients in a virtual environment
• Strong documentation and computer skills
• Experience working within an electronic medical record system
• Excellent communication, organization, and time management skills
• Ability to work independently while maintaining productivity standards
• Professional, dependable, and adaptable approach to patient care

Preferred Qualifications

• Experience with annual wellness visits or Medicare populations
• Athena experience preferred
• Previous phone triage, care coordination, or population health experience
• Value-based care experience a plus
