Wellness & Recovery Specialist (US)-6

Location: This is a virtual/remote position working from your home. Ideal candidates will live within 50 miles of one of our Elevance Health PulsePoint locations.

The Wellness & Recovery Specialist is responsible for care coordination and/or care management activities focused on the Wellness and Recovery of members.

How you will make an impact
  1. Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.
  2. Develops and implements provision of onsite psychiatric discharge planning education at Recovery and Resiliency sites.
  3. Collaborates with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.
  4. Acts as a resource for staff on decision making and problem solving.
  5. Initiates and maintains contact with assigned individuals and providers to determine member's response to services.
Minimum Requirements

Requires AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role; or any combination of education and experience, which would provide an equivalent background.

Preferred Skills, Capabilities and Experiences:
  1. BA/BS preferred.
  2. Knowledge of care-coordination and case management concepts, strongly preferred.
  3. Experience in health coaching and motivational interviewing techniques, preferred.
  4. One year completed as a certified Peer Support Specialist or have equivalent related experience, preferred.
  5. Telephonic customer service skills, strongly preferred.
  6. Experienced in the recovery process and in facilitating recovery through established mental health processes, preferred.
  7. Experienced in teaching and engaging clients in basic problem-solving strategies for self-directed recovery, preferred.
  8. Ability to recognize signs and symptoms of mental illness and substance use disorders, preferred.
  9. Experienced in utilizing community resources for independent living and teaching these skills to individuals with severe mental illness, preferred.
  10. Experienced in establishing and sustaining self-help and educational groups by incorporating input from mental health consumers, preferred.
  11. Proficiency using multiple computer platforms simultaneously with excellent typing skills and attention to detail, preferred.
