Job summary

The Community Health Center of Southeast Kansas (CHC/SEK) is hiring a Health Promotion Specialist to advance wellness initiatives across adult and pediatric populations. The role focuses on outreach for preventive services, chronic disease support, and improving care gaps within the CHC/SEK system.

In this position you will coordinate outreach, verify demographics in the EHR, and schedule appointments while ensuring HIPAA compliance.

Qualifications

• Must possess good communication skills and be comfortable speaking on the phone.
• Ability to listen carefully and document information accurately.
• Proficient with Word/Excel and EHR software.
• Experience with appointment scheduling is a plus.
• Medical terminology knowledge is a plus.

Responsibilities

• Identify patients due for gaps in care using Population Health tools.
• Outreach to patients for preventive services to close gaps.
• Verify patient demographics and update the EHR and Population Health tools as needed.
• Communicate with patients about items needed for registration and appointments.
• Schedule and reschedule appointments in the EHR per guidelines.
• Document outreach outcomes in EHR and Population Health tools.
• Conduct Health Risk Assessments prior to MAWV; document responses.
• Collaborate with Population Health and Primary Care nursing staff as needed.
• Assist with data collection for quality improvement and payer audits.
• Participate in team meetings and training for ongoing education.
