Job summary

Community Health Center of Southeast Kansas (CHC/SEK) is seeking a Health Promotion Specialist to promote wellness initiatives and conduct outreach for preventive services, cancer screenings, and chronic disease care. The role emphasizes using population health tools, documenting workflows, and supporting quality improvement efforts.

The ideal candidate has strong communication skills, basic healthcare knowledge, and experience with scheduling, EHRs, and patient contact.

Qualifications

• High School diploma or equivalent required.
• At least 1 year of experience in a healthcare setting, call center, data entry, or similar position required.
• Experience with appointment scheduling preferred.
• Medical terminology training/experience preferred.

Responsibilities

• Identify patients who are due for gaps in care using Population Health tools.
• Outreach to patients due for preventive services to close gaps in care, such as annual wellness visits and age-based screenings.
• Verify patient demographics and update the EHR and Population Health tools as needed.
• Communicate with patients regarding necessary items for registration and appointments.
• Schedule appointments accurately in the EHR, including rescheduling as needed.
• Document outreach outcomes in the EHR and Population Health tools.
• Conduct Health Risk Assessments prior to MAWV and document responses.
• Identify additional patient needs and escalate to Population Health/Primary Care staff as needed.
• Request relevant patient records from external facilities to close gaps in care.
• Monitor lists of no-shows/cancels/reschedules and outreach to reschedule as appropriate.
• Follow up with patients on cancer screening orders and identify barriers.
