Qualifications

• Experience: Supervisor Experience; No Experience.
• Licenses & Certifications: RN.

Responsibilities

• Assess patient preferences and barriers to involvement in care.
• Develop and document individual care plans with defined goals.
• Communicate plan of care across the continuum of care.
• Promote respect, equity and empathy for diverse populations.
• Collaborate with interprofessional team and family to implement care.
• Integrate data from the interprofessional team and use critical thinking.
• Advance nursing practice through community health events and quality initiatives.
• Update plan of care in EMR as conditions change.
