Job summary

Outreach Development Corp. in New York is seeking a Wellness Liaison to extend the activities of the Manager of Community Partnerships.

The role performs intakes onsite, conducts educational workshops, and supports referrals across Suffolk County/Long Island, ensuring a warm transition to Outreach programs. You will engage prospective participants, maintain contact through intake, and work to meet monthly and quarterly targets.

Qualifications

• 1-2 years of work experience.
• Experience with at-risk populations is a plus.

Responsibilities

• Extend activities of the Manager of Community Partnerships by performing intakes and preparing/leading educational workshops.
• Provide onsite screenings/intake at hospitals, courts, housing sites and other locations as needed.
• Maintain professional presence at referral sources and deliver excellent customer service.
• Identify potential clients for Outreach programs and services in collaboration with the Manager of Community Partnerships.
• Complete comprehensive intake and required documentation.
• Achieve monthly/quarterly targets for Wellness Liaison and ensure smooth admission to Outreach programs.
