Sunrise Senior Living in Montgomery County, PA is seeking a Wellness Nurse RN to oversee resident health, support medication safety, and ensure regulatory compliance across the community. You will conduct assessments, monthly wellness visits, and follow-ups on falls and hospital returns.

Qualifications

• Active RN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, regulatory, and communication skills.

Responsabilités

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication program compliance.
• Partner with families, physicians, and the Resident Care Director.
• Promote infection control and survey readiness.
• Provide clinical guidance and training to care staff.
