Spectrum Retirement Communities, LLC is seeking a Wellness Nurse for a part-time overnight shift in Hanover Township, IL. You will monitor residents' health, support community nurses, and ensure care aligns with individual care plans.

The ideal candidate is a graduate of an accredited practical nursing program, holds an active LPN or RN license, and has 3+ years of geriatric assessment experience. We offer benefits for part-time staff and opportunities for paid time off and 401(k).

• Current nursing license and state registration (LPN or RN).
• Three years' experience in assessment and geriatric nursing preferred.

Responsabilités

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assistance with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.

Schedule: Part-Time Overnight Shift

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

In this position, your main responsibilities will include:

• Assist in coordination of the resident admission process including screening, assessment, and care needs

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three (3) years'experience in assessment and geriatric nursingpreferred
• A big smile, eagerness to learn, and compassion for our residents
