SCOPE OF POSITION:

The Wellness Coordinator ensures that the day-to-day wellness needs of Ernest P. Barka Assisted Living Community residents are met by carrying out personal care treatments and medication administration. Acting as part of an interdisciplinary team, Wellness Coordinators ensure residents receive consistent, high-quality services and help residents achieve their long and short-term wellness goals. Further, they act as a resource for residents, families, and providers and are encouraged to actively build authentic relationships as part of the overall mission of delivering person-centered care.


A DAY IN THE LIFE:

We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our Wellness Coordinators follow the care plans as determined by the Wellness Manager, and further ensure quality care by assisting with admissions, individualizing and proactively managing personal service plans, administering medication/treatments, taking vital signs, temperature, blood pressure, pulse, and respiration, and documenting all services in accordance with Rockingham County policy and State/Federal Regulations. As part of our team-oriented environment, the Wellness Coordinators keep open communication with the Wellness Manager regarding changes in resident status to include condition, environment, or personnel concerns. Further, they contribute to the team effort by volunteering to help others, coaching staff as necessary and attending community events.


REQUIRED EDUCATION AND EXPERIENCE:
  • Minimum of three (3) years full-time or equivalent experience in a Health Care setting with a minimum of one (1) year in a supervisory role; or
  • An equivalent combination of work and experience will be considered.
  • Licensed nurse in the state of New Hampshire (LPN/RN) required.

STATUS:

Per Diem / Non-Exempt
