Job summary

Centerstone in Illinois is seeking a full-time Behavioral Health Coach to provide support, education and coaching for clients, promoting wellness, recovery and skill-building while helping individuals advocate within their healthcare systems.

You will conduct intake, perform assessments, connect clients with community resources, and provide transportation and ongoing case management to support recovery goals.

Qualifications

  • Associate’s degree and 1 year of participation in own personal recovery process.
  • Experience providing mental health services preferred.
  • Ability and willingness to obtain credentialing if required.

Responsibilities

  • Establishes relationships and applies coaching skills to transform client goals into action.
  • Provides group and/or individual training to improve client functioning and health-related issues.
  • Assists clients with developing WRAP and wellness goals in collaboration with providers.
  • Empowers clients to self-manage chronic conditions using motivational interviewing.
  • Links clients to community resources and coordinates with other departments.
  • Provides ongoing case management including assessment, planning, linking, monitoring, referral and advocacy.
  • Provides transportation assistance to help clients achieve wellness goals.
  • Conducts intake and completes necessary documentation; writes assessment summaries.
  • Assists with discharge planning and follow-up services.

JOB DESCRIPTION: SUMMARY OF POSITION Provides support services, information, education and support for clients. Promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, assisting them in developing a natural support systems, and coaches clients to advocate for themselves within their healthcare systems.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Establishes relationships and practices core coaching skills to transform client values/desires into action and maintain lasting change over time.
  • Provides group and/or individual training/interventions focusing on improvement of the client’s functioning and health-related issues (e.g., symptom/medication management, life skills, healthy eating, weight management, smoking cessation, recognizing the dangers of alcohol and other drugs, community reintegration, etc.).
  • Provides wellness consultation. Assists clients with developing their Wellness Recovery Action Plan (WRAP). Works in consultation/collaboration with clients, family members and other providers to implement plan.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals. Assists clients with finding solutions utilizing motivational interviewing and finding motivation to complete their individualized recovery plan to achieve wellness goals. Assists clients in overcoming any barriers to goals, including access to care. Assists with navigating health care systems.
  • Links clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
  • Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.
  • Provides transportation assistance to help clients achieve and maintain wellness goals and initiatives.
  • Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessment and write assessment summaries, as assigned.
  • Assists service recipient in making discharge plans and provide follow up services as assigned.
KNOWLEDGE, SKILLS & ABILITIES
  • Effectively communicate via written, verbal, in person and virtual methods.
  • Skill in customer service/experience techniques.
  • Skill in problem-solving.
QUALIFICATIONS

Education Level: Associate’s degree and 1 year of participation in own personal recovery process in order to care for self. Participation in personal recovery process or work experience equivalent to 3 years required. Must have the ability to share their recovery journey.

Years of Experience: Experience providing mental health services preferred.

Certification/Licensure: Ability and willingness to obtain credentialing if required.
