Qualifications

• Must be self-motivated, creative with positive attitude.
• Must work well with families, partners, and the community.
• High School Diploma or GED required.

Responsibilities

• Develop and implement a well-rounded wellness program for Long Term Care, Memory Care or Rehab patients.
• Timely completion of assessments, care plans and related paperwork.
• Ensure compliance with local, state, and federal guidelines for programming.
• Bring energy and fun to activities; perform other duties as assigned.
