Iowa Home Care seeks an experienced Welcome Home RN to lead Start of Care admissions in the Des Moines area. You’ll perform comprehensive SOC assessments, complete OASIS, and coordinate with physicians and the care team to build an effective home-based plan.

Qualifications

• Experience with Start of Care (SOC) admissions.
• OASIS experience or familiarity preferred.
• Knowledge of Medicare home health requirements and documentation.

Responsibilities

• Complete SOC admissions for new clients.
• Perform OASIS assessments and admission documentation.
• Conduct comprehensive nursing assessments and med reconciliation.
• Identify client needs (clinical, functional, safety, social, support).
• Develop individualized care plans for safe home care.
• Coordinate orders with physicians and other providers.
• Educate clients and families about care and plan.
• Collaborate with nursing, therapy, social work, aides, scheduling, and leadership.
• Provide thorough handoff to the RN Case Manager and team.

We call it our Welcome Home Department - because we believe the beginning of home health care should feel exactly that way.We're looking for an experienced, compassionate Registered Nurse to join our Welcome Home team and specialize in Start of Care admissions for clients throughout the Des Moines area.This is a great opportunity for an RN who loves assessment, OASIS, care planning, patient education, and putting all the pieces together - but may be ready for something different from a traditional ongoing case-management role.Your specialty will be getting care started right.What You'll DoAs our Welcome Home RN, you will:Complete comprehensive Start of Care (SOC) admissions for new Iowa Home Care clients.Complete OASIS assessments and required admission documentation.Perform comprehensive nursing assessments and medication reconciliation.Identify clinical, functional, safety, social, and support needs.Get to know each client and family, including their goals, priorities, and what matters most to them.Develop individualized plans of care designed to help clients safely and successfully remain at home.Coordinate with physicians and other providers to obtain necessary orders.Identify appropriate disciplines, services, visit frequencies, and clinical priorities.Educate clients and families about their care and what they can expect from Iowa Home Care.Collaborate with nursing, therapy, social work, home health aides, scheduling, and clinical leadership.Provide a thorough clinical handoff to the RN Case Manager and other clinicians who will continue the client's care.Help ensure every new client has a smooth transition into Iowa Home Care.Our goal is simple: when you hand a new client over to their ongoing clinical team, they should understand the client, understand the plan, and have everything they need to provide exceptional care.What We're Looking ForRequiredCurrent Iowa Registered Nurse (RN) licenseStrong clinical assessment and critical-thinking skillsExcellent communication and organizational skillsAbility to work independently and manage your day effectivelyCommitment to timely, accurate, high-quality clinical documentationA genuine desire to make clients and families feel comfortable and confident receiving care at homePreferredPrevious home health experiencePrevious OASIS experienceExperience completing Start of Care assessmentsKnowledge of Medicare home health requirementsAdmissions, care-transition, discharge-planning, or case-management experienceDon't check every preferred box?
