Job Title: Home Health Admission & OASIS Registered Nurse (RN) - Weekends Only

Compensation: $200 per visit + mileage reimbursement

Position Summary

Are you looking for your next home care career as a Registered Nurse? Home Health Care Incorporated has a fantastic opportunity for an RN to perform admission and start of care visits around the Twin Cities on weekends. If you have a passion for home care, we encourage you to apply!

Responsibilities

• Admission/OASIS Visits:
  
  

Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro.

• Client Evaluation:
  
  

Evaluate and assess clients for home health services and create their plan of care.

• Service Recommendation:
  
  

Recommend services from other disciplines that have not yet been ordered by the physician.

• Collaboration:
  
  

Collaborate with clients, families, physicians, and other members of the home care team.

Qualifications

• Licensed as a Registered Nurse in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
• Prior Home Health Nursing experience.
• Proficiency with Medicare & OASIS documentation.
• Demonstrated ability to work autonomously.
• Excellent at planning, organizing, and communicating.
