Crowne Health Care in Alabama is seeking a Wound Care / Treatment Nurse (LPN or RN) for a Baylor Weekend position to join our geriatric care team. You will assess wounds, provide treatment, document care, and coordinate with the interdisciplinary staff.

The ideal candidate has an Alabama LPN/RN license, wound care experience or certification, strong assessment and communication skills, and a compassionate, team-focused approach. Training and certification support available.

Qualificações

• AL licensed LPN or RN is required.
• Wound care experience or certification preferred.
• Certification training paid by the facility is available.
• Strong assessment, documentation, and communication skills required.
• Long-term care experience preferred.

Responsabilidades

• Assess residents' skin integrity and wounds and provide follow-up care.
• Perform and document ordered wound care and treatments per physician orders.
• Provide treatment for acute and chronic wounds, ostomies, stomas, skin tears, pressure injuries.
• Monitor skin condition and promptly communicate changes to nursing staff and providers.
• Work with licensed staff to identify and address risks such as skin breakdown and weight loss.
• Coordinate treatments with residents' daily schedules and other departments.
• Assist with development, implementation, and follow-up of resident care plans related to wounds and skin.
• Maintain accurate and timely documentation per policies and procedures.
• Communicate with physicians, families, nursing staff, therapy, dietary, and other members of the interdisciplinary team as needed.
• Participate in staff meetings, in-services, quality assurance activities, and wound prevention initiatives.
• Provide a multidisciplinary approach to wound management and treatment to promote healing and maintain high resident care.
