Responsibilities

• Provide direct, individualized nursing care based on scientific principles
• Coordinate with healthcare team to assess, plan, implement, and evaluate patient care
• Administer medications and record treatments accurately
• Monitor and record patient vital signs and medical information
• Operate and interpret specialized medical equipment
• Respond to emergencies and participate in discharge planning
• Maintain timely and accurate documentation

Requirements

• Current LPN or RN license in State
• Up-to-date CPR and First Aid certification
• Minimum one year of experience in Long Term Care preferred
• Knowledge of Matrixcare preferred
