Job summary

BeLoved Hospice is seeking compassionate LPNs/RNs for after-hours care in the Las Vegas area. You will provide direct palliative care, communicate patient needs to the hospice team, and follow a medically approved care plan.

The role emphasizes after-hours triage, coordination with the interdisciplinary team, and maintaining high standards of patient comfort and dignity in a flexible, supportive work culture.

Qualifications

JOB SUMMARY

Provides care to patients after standard office hours. Provides direct palliative care to terminally ill patient and their families while functioning within a medically approved plan of care and demonstrates proficiency in triaging patient emergencies both in person and on the phone while effectively communicating patient needs and concerns to the correct member of the hospice team.

DUTIES & RESPONSIBILITIES
  • Follows an individualized Plan of Care established by the interdisciplinary team for each patient.
  • Manages emergency care to patients encompassing coordination of all hospice disciplines during non-business hours and reports issues and significant changes to the RN Case Manager.
  • Immediately reports any changes to the Physician, RN Case Manager or appropriate hospice team member for the patient.
  • Reports unusual conditions, observations and pertinent information to the nurse.
  • Provides intervention and support as needed related to terminal illness and loss within Scope of Practice.

  • Assesses and address depression and fear associates with terminal illness
  • Facilitates communication among patient, family and hospice team
  • Capable of giving adequate report on patient’s condition to the Hospice Medical Director, members of the interdisciplinary team, other nurses and supervisors, as needed.
  • Recognizes signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by the RN Case Manager or Hospice Physician. Coordinates medications, DME, and supplies necessary to complete the after-hours admission and keep the patient comfortable until business hours.
  • Provides personal care as assigned by the Interdisciplinary Team, Chief Medical Officer, Hospice Physician, Director of Nursing, which may consist of but not limited to:
  • Focused/limited nursing visits
  • Telephonic triage
  • Provide comfort measures
  • Insertion/maintenance/removal of foley catheters
  • Insertion/maintenance/removal of various intravenous and subcutaneous catheters for pain control/comfort per scope of practice and within individual’s competency set
  • Evaluating IV site and check IV function
  • Medication administration (via oral, transdermal, rectal, enteral, IV, SQ route) per scope of practice and within individual’s competency set
  • Obtains accurate vital signs
  • Performs standard diagnostic procedures including phlebotomy, venepuncture, and doppler.
  • Assist with ambulation/transfers
  • Dressing changes (including, but not limited to central line, IV, or SQ dressings, percutaneous dressings, ostomy dressings, tracheostomy dressings)
  • Wound care
  • Other duties as assigned.
JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  • Preferredminimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. Training will be provided for all candidates, regardless of experience.
  • Maintains an active LPN license in the state of Oregon
  • Maintains a CPR/BLS certification
  • Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.
  • Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
