Weekend Start of Care (SOC) Nurse (RN) New

Overview
Our client is seeking a Full-Time Weekend Start of Care (SOC) Nurse (RN) to promote the delivery of coordinated, comprehensive care to patients through assessment, planning documentation, and provision of direct nursing care. The ideal candidate will have an Associate Degree in Nursing, at least two years of RN experience, and be CPR certified. Responsibilities include accurately completing assessments, administering skilled nursing care, and collaborating with other healthcare professionals to ensure patient care needs are met.

Key Responsibilities
  1. Accurately completes required start of care documentation on new admissions, post-hospitalization.
  2. Assesses the patient for home telemonitoring, incorporates telehealth into plan of care, and ensures notification of patient admission to the telehealth program to the telehealth department in established timeframes.
  3. Develops and implements the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient.
  4. Observes signs and symptoms and reports, to the physician, reactions to treatments including drugs and changes in the patient's physical and emotional condition.
  5. Teaches, supervises, and counsels the patient and family members regarding the nursing care needs and initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
  6. Assesses and assigns Home Health Aides as appropriate by providing a written plan of care and specific duties the Aide is to perform for each individual patient.
  7. Initiates preventative and rehabilitative nursing procedures, as appropriate, for the patient’s care and safety, according to the Agency’s policies and procedures.
  8. Communicates with supervisor and, as indicated to other members of the home care team, concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization.
  9. Attends and participates in office staff meetings, patient care conferences, record audits, utilization review, performance improvement, in services, workshops, and seminars.
  10. Satisfies CEU requirements of the State Board of Nursing.
  11. Keeps up-to-date on reimbursement criteria and charting needs for all patients.
  12. Strives to improve nursing care through continuing education, attendance at workshops and conferences, active participation in professional and related organizations, and individual research and readings.
  13. Adheres to the Agency’s personnel policies and Compliance Plan.
  14. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
  15. Respects patient rights, and property.
  16. Must call in report daily to Supervisor or scheduler.
  17. Contacts the Nurse Supervisor to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days. Immediately reports any schedule change or inability to complete assignment as scheduled.
  18. Accepts changes/additions to schedule during staffing difficulties when necessary.
  19. Organizes daily visit schedule to minimize travel time and mileage. Maintains productivity according to work status. You will be expected to travel where business needs are located in order to reach your weekly productivity.
  20. Must meet weekly productivity points per week. Equivalent of three (3) SOCs or ROCs per day, for a total of six (6) points.
  21. Must work holiday as required.
  22. Untimely or poor documentation is subject to disciplinary action, up to and including termination.
  23. Must travel to where business needs are located in order to reach productivity. Our client will always do their best to keep clinicians within a reasonable territory.
  24. Performs other duties as assigned by supervisor.
Schedule and Shift Details

Schedule: every Saturday & Sunday + two weekdays for four 10-hour days; prefers Friday – Monday or Saturday – Tuesday

Travel

Must travel to where business needs are located in order to reach productivity. Our client will always do their best to keep clinicians within a reasonable territory.

Required Qualifications
  1. Associate degree or higher
  2. Valid Certified Cardiac Rehabilitation Professional (CCRP) certification/license
  3. Valid Registered Nurse (RN) certification/license in Illinois (IL)
  4. Valid Regular Driver License (DL) certification/license
  5. Valid American Heart Association Basic Life Support (AHA BLS) and American Heart Association Advanced Cardiac Life Support (AHA ACLS) certifications/licenses
  6. Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
  7. Are you okay with the required travel for this position?
