Home Health Care, Inc. is seeking an experienced Home Health RN to focus on Start of Care admissions during the weekends. You will assess patients in their homes, complete SOC and OASIS documentation, and begin care planning with the clinical team.

You must hold an active Minnesota RN license and CPR certification. This role offers a weekend schedule and mileage reimbursement to support travel between patient visits.

The Start of Care nurse has an incredibly important role in home health.

You are often the first clinician entering the patient's home. You'll assess their condition, identify immediate concerns, determine what services are needed, educate the patient and family, and establish the clinical foundation for the entire episode of care.

For an experienced home health nurse, it's an opportunity to use the full range of your assessment, OASIS, care coordination, and patient education skills.

What You'll Do

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.

• Excellent clinical assessment and documentation skills.
  
• Confidence working independently in patients' homes.
  
• Valid driver's license, active auto insurance, and reliable transportation.
