Responsibilities

• Coordinate patient care activities, including developing and implementing patient care plans.
• Ensure compliance with regulatory standards, including HIPAA and Medicare/Medicaid guidelines.
• Participate in ongoing education and professional development opportunities to stay current with latest clinical practices.
• Collaborate with inter-disciplinary team members to develop and implement patient care plans.
• Provide patient education and support to patients and their families.
• Document patient care activities and maintain accurate records.
