You will perform assessments, medication administration, and treatments while coordinating with the care team to implement individualized plans and maintain thorough documentation. A strong emphasis on patient-centered care and quality improvement is required.

Qualifications

• Current RN licensure in New York.
• Ability to work every weekend 12 hour or 16 hour shifts.

Responsibilities

• Provide nursing care to residents on weekends, including assessments, medication administration, and treatments.
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans.
• Manage and coordinate resident care activities, ensuring that needs are met and expectations are exceeded.
• Demonstrate a commitment to person-centered care, prioritizing the physical, emotional, and social well-being of each resident.
• Participate in quality improvement initiatives, contributing to the continuous improvement of our care services.
• Maintain accurate and detailed records of resident care and communicate effectively with the healthcare team.
• May pass medications.
