Job summary

Enhabit Home Health & Hospice in Des Plaines, IL is seeking a Full-time weekend RN to deliver skilled care to patients in their homes. The role requires Saturday and Sunday coverage with flexible scheduling and adherence to the physician’s orders and the established plan of care.

You will educate patients and families, document care, and collaborate with the health care team to ensure high-quality home-based services. A valid RN license and CPR certification are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
