Qualifications

• Currently licensed RN in state of practice.

Responsibilities

• Deliver quality nursing care focused on outcomes using the nursing process.
• Identify needs and input for discharge planning and follow-up.
• Assist with development of individualized care plans with the care team.
• Implement care plans consistently and cost‑efficiently with resident-centered outcomes.
• Collaborate with physicians and other health professionals; share plan changes.
• Maintain optimum resident care and comfort with current nursing practices.
