Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) to join our weekend on-call team in Texas. You will case manage, provide clinical directions to the interdisciplinary team, and implement the plan of care in home health settings.

The ideal candidate has an RN license in Texas, CPR certification, and at least 1 year of RN experience (2 years preferred). Reliable transportation and a valid auto liability insurance are required.

Qualifications

• Graduate of an accredited diploma, ADN, or BSN program.
• Current Texas RN license and driver's license.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with current auto liability insurance.
• One year RN experience; two years preferred.

Responsibilities

• Covers weekend on-call schedule.
• Case manages and provides clinical directions to the interdisciplinary team, physician, and family.
• Implement/develop/document the plan of care.
• Provide care utilizing infection control measures that protect both the staff and the patient.
• Assess home health patients to identify the physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third-party payer.
• Promote Agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Supervise and provide direction to the home health aide and LVN/LPN.
• Demonstrate commitment, professional growth, and competency.

• One year’s experience as a Registered Nurse, two preferred.
