The Segue Residential Transition Program assists individuals in transitioning out of state psychiatric hospitals. Clients are clinically stable and ready to leave the hospital. The services help clients manage their illnesses and develop independent living skills.

What to expect

This is a direct care role supporting mental health treatment. You will provide a safe and therapeutic environment that promotes independence and wellbeing. Pay rate: $18.50/hr.

Responsibilities under close supervision include:
  1. Participating in initial assessment, intervention, and re-evaluation of client needs.
  2. Working one-on-one with clients to learn or regain skills for community and independent living.
  3. Coordinating care, including monitoring medication compliance.
  4. Providing transportation, attending medical appointments, advocating, and documenting follow-up instructions.

Building rapport with clients, becoming part of their trusted support system, and celebrating their progress are key aspects of the role.

Training and Support

All staff will receive thorough training and mentorship from senior staff members.
