Job Description

Mass General Brigham Healthcare at Home serves patients from rehabilitation after illness or injury to chronic disease management. Responsibilities
  1. Provide quality, clinical services to patients in various community settings.
  2. Deliver patient care aligned with organizational philosophy, policies, and community health standards.
  3. Manage admissions and case responsibilities according to MGB Healthcare at Home policies, ensuring thorough and timely electronic documentation and interdisciplinary communication.
  4. Offer skilled nursing assessments, teaching, treatment, documentation, and care coordination, demonstrating high-quality customer service and financial awareness.
Qualifications and Experience
  • Graduate of an approved School of Nursing.
  • Current Massachusetts license as a Registered Nurse.
  • Minimum of one-year previous nursing experience in an acute care setting; prior home care experience preferred.
  • Ability to travel within MGB Healthcare at Home geographic area.
  • Proficiency with computer software; knowledge of handheld patient care devices preferred.
  • Strong interpersonal, customer service skills, and ability to work with diverse populations.
  • Effective communication skills in writing, verbal, and electronic formats.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Organizational and prioritization skills, with adaptability to changing situations.
  • Ability to work independently, be self-directed, and handle unpredictable circumstances.
