Job summary

Sequoia Hospice is seeking an experienced Weekend Registered Nurse (RN) to join our Hospice Care team in California. The role involves planning, coordinating, and delivering high-quality home-based hospice care on weekends, with an emphasis on patient comfort and family education.

You will hold an active CA RN license, CPR certification, and have at least 2 years of nursing (hospice or home health preferred).

Qualifications

• Active CA RN license required.
• Minimum 2 years of nursing experience (hospice or home health preferred).
• Current CPR certification and valid driver’s license with insured vehicle.

Responsibilities

• Plan, organize, and direct hospice care in the home setting.
• Serve as case manager and ensure quality, compassionate care.
• Complete initial and ongoing patient and family assessments.
• Develop and revise individualized hospice care plans.
• Administer medications and treatments as prescribed.
• Educate and counsel patients and families on hospice care.
• Coordinate care for assigned caseload.

Weekend Registered Nurse (RN) – Hospice

Pay: $60.00 – $67.00 per hour

Schedule: Every Weekend, 8 hour shifts

Coverage Area: Alameda County, Santa Clara County, Peninsula

We’re growing and looking for an experienced Registered Nurse (RN) to join our Hospice Care team!

What You’ll Do

As a Hospice RN , you will plan, organize, and direct hospice care while educating patients and families. You’ll serve as a case manager and ensure quality, compassionate care in the home setting.

Key Responsibilities:

• Complete initial and ongoing patient and family assessments
• Develop and revise individualized hospice care plans
• Provide professional nursing care using the nursing process
• Administer medications and treatments as prescribed
• Educate and counsel patients and families on hospice care
• Coordinate care for your assigned caseload

What We’re Looking For

• Active CA RN License
• Minimum 2 years of nursing experience (hospice or home health experience preferred)
• Strong clinical, communication, and problem-solving skills
• Current CPR certification
• Valid driver’s license, insured vehicle in good condition
• Ability to work independently and manage priorities
