Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience; Home Health experience is a plus.
• Strong critical thinking and case management abilities, patient-centered care.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Be a clinical leader, delivering high-quality, personalized nursing care in the patient's home.
• Craft individualized care plans and dynamically adjust them to promote independence.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Mentor and guide licensed practical nurses and aides to foster a cohesive care team.
• Utilize technology to optimize care delivery and maintain precise documentation.
