Job summary

CommonSpirit Health at Home seeks a PRN Home Health Registered Nurse to serve Leadville, Fairplay, and Frisco areas, delivering high-quality in-home nursing care with a 1:1 patient-clinician focus.

You will lead clinical care, educate families, collaborate with physicians and therapists, and leverage technology to maintain precise documentation and care coordination. This role offers flexible PRN hours in the Colorado region.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader. Provide high-quality, personalized nursing care in the patient's home.
• Craft individualized care plans and adjust them as needed for independence and well-being.
• Collaborate with a multidisciplinary team–physicians, therapists, and support staff–to ensure holistic care.
• Educate patients and families, providing crucial teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Innovate with technology and maintain precise documentation.
