Job summary

Salem Health & Rehabilitation Center in Salem, VA is seeking a caring Physical Therapist who is licensed in Virginia or a graduate from an approved PT program and working toward licensure. Join our in-house team of rehab professionals delivering therapy in a state-of-the-art rehab gym to help residents regain independence and return home.

The Physical Therapist evaluates residents, develops treatment plans, obtains physician approval, and participates in discharge planning.

Qualifications

• Virginia Physical Therapy License is required.
• Graduate from an approved Physical Therapist program and actively pursuing licensure.

Responsibilities

• Evaluates residents and develops effective treatment plans.
• Obtains approval for services from referring physician.
• Participates in discharge planning and documentation.
• Supervises Physical Therapist Assistants and Aides.
• Screens residents for potential benefit from PT intervention.
