Job summary

Lakeview Village, Inc. is seeking a Physical Therapist to provide weekend Home Health services on a PRN basis.

The role involves evaluating residents, creating treatment plans, and coordinating with a multi-disciplinary team to support independence and prevent decline. Eligibility requires graduation from an APTA-accredited PT program and licensure in Kansas, with strong communication and teamwork skills to work with residents and families in a compassionate setting.

Qualifications

• Must be a graduate of an American Physical Therapy Association accredited school and licensed to practice in Kansas.
• Demonstrates good communications skills, including reading, writing, speaking, and English fluency.
• Empathetic approach to the elderly and able to work in a team with residents, relatives, staff, and others.

Responsibilities

• Consults with physician, resident, and family to establish goals and develop a PT treatment plan.
• Assesses evaluations, administers treatments, and directs assistants in therapies.
• Monitors progress, adjusts equipment and plans to support independence and discharge planning.
