BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a PER DIEM WEEKEND opportunity providing home health visits for our senior living office in RALEIGH, NC. The office services adult and geriatric clients across WAKE & DURHAM counties.

• Must be available 2 weekends per month

Home Health Experience Required!

As a home care Physical Therapist, you will be a key part of a multidisciplinary team delivering rehabilitative and skilled nursing care, enabling clients to remain at home.

Responsibilities include:

1. Making home visits within designated territories.
2. Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments.
3. Implementing protective interventions to prevent wounds and promote healing using evidence-based dressings and topical agents.
4. Developing and executing personalized care plans, including manual exercises, gait training, and balance interventions.
5. Continuously assessing and updating care plans and collaborating with nursing staff.
6. Educating clients and their families on rehabilitative activities to promote health, safety, and independence.
7. Documenting observations and interventions accurately using a touch pad tablet.

Qualifications:

1. Current North Carolina PT license.
2. At least one year of recent PT experience.
3. Graduation from an accredited physical therapy program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; EMR experience preferred.
