Position: Per Diem Weekend Physical Therapist (PT)

Requirements: Home health experience required; availability for 2 weekends per month

Responsibilities

1. Make home visits to clients within designated territories.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual therapies and gait training.
5. Assess and collaborate on multidisciplinary care plans.
6. Educate clients and family members on rehabilitative activities.
7. Document all client interactions and interventions accurately using electronic medical records.

Qualifications

1. Current North Carolina Physical Therapist (PT) license.
2. Minimum of one year recent home health experience as a PT.
3. Graduation from an APTA-approved program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; EMR experience preferred.
