Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs.
• Provide age-appropriate education for clients and caregivers.
• Supervise and assume responsibility for care provided by identified team members.
• Collaborate with internal and external team members to improve outcomes.
• Develop and update the formal plan of care for each patient.
