Job summary

You will develop individualized care plans with the member, caregiver, and PCP, and coordinate with the interdisciplinary team to optimize health outcomes.

Qualifications

• Nursing background with ability to apply CMS/MCG guidelines.
• Strong communication with patients, families, and providers.
• Experience with managed care and utilization review processes.
• Proficient in HIPAA privacy and patient confidentiality.

Responsibilities

• Conduct intake and provide case management for members with chronic conditions.
• Develop individualized care plans with member, caregiver, and PCP.
• Coordinate ICT meetings and communicate care plans with the team.
• Document assessments and care plans in the Care Management System.
• Ensure compliance with HIPAA, payroll, and attendance policies.
