Interim HealthCare Hospice & Palliative Care is seeking a Weekend On-Call Registered Nurse to join our team in the Erie area. You will be called to provide compassionate, patient-centered care during weekend on-call shifts from Friday afternoon through Monday morning.

The role emphasizes developing and evaluating care plans with the care team, ensuring timely communication with providers, and delivering comfort and dignity to patients.

Qualifications

• Active RN license in the applicable state.
• Current CPR/FA certification.
• One year hospice or similar setting preferred.

Responsibilities

• Oversee implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers as needs evolve.
