Job summary

Phoenix Home Care and Hospice is seeking a Home Health RN to be the first point of contact between patients and hospitals in Springfield/Branson, MO and surrounding areas. You will assess patients, interpret diagnostics, and formulate individualized care plans per physician orders, collaborating with a multidisciplinary team.

We offer AI-assisted charting during visits to support work-life balance and opportunities for orientation and training. Weekend on-call is required.

Qualifications

• Active RN license in Missouri or Compact state.
• Home Health or Critical Care experience preferred.
• Valid driver's license and insured vehicle.

Responsibilities

• Assists in development, review and revision of the plan of care.
• Provides nursing care according to physician's plan of treatment and individual plan of care.
• Observes, records and reports reactions to treatment and changes in client’s condition.
• Provides ongoing instruction and supervision of client, family, or other health team personnel.
• Participates in education programs, quality assessment activities, nurse meetings and other staff meetings.
• Monitor and record patient’s condition and document provided care services via electronic charting.
