Job summary

Apploi is seeking a Nurse Practitioner (Skilled Nursing) to provide internal medicine services at a Skilled Nursing Facility on weekends. You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitalizations and manage the care of short- and long-term residents.

The role requires Florida APN licensure with an active DEA number, a Master's degree, and prior nursing or medical experience.

Qualifications

• APN license in Florida and active DEA number.
• Previous experience in nursing or other medical fields.
• Strong communication and leadership skills; able to work in fast-paced healthcare settings.

Responsibilities

• Provide internal medicine services for SNF on weekends.
• Collaborate with interdisciplinary team and physicians to reduce weekend hospitalizations.
• Conduct admission, discharge, and condition-change evaluations.
• Prescribe treatments under established protocols and manage acute and chronic illnesses.
• Perform comprehensive physical examinations and develop patient management policies.
• Coordinate referrals and communicate with attending physicians.
