Job summary

Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead internal medicine coverage at a Skilled Nursing Facility on Saturdays and Sundays. You will work with the interdisciplinary team and physicians to reduce hospitalizations on weekends, while evaluating new admissions and managing patient care.

The role requires an APRN with Texas licensure, active DEA registration, and a Master’s degree, along with strong communication and leadership abilities in a fast-paced healthcare

Qualifications

• Advanced Practice Licensure (APRN) with a Texas license.
• Active license and DEA number required.
• Previous nursing or medical field experience preferred.

Responsibilities

• Provide internal medicine services as SNF leadership on weekends.
• Collaborate with interdisciplinary team and physicians to reduce hospitalizations.
• Evaluate new admissions and coordinate discharge visits.
• Perform comprehensive physical examinations and prescribe per protocol.
• Develop patient management policies and facilitate referrals.

Nurse Practitioner (Skilled Nursing)

Major Responsibilties

• As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.

• You will collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends.

• You will work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians

• Evaluation of new-admission visits

• Coordination of discharge visits

• Evaluations for patients exhibiting changes in condition

• As a SNF-ist, you will provide types of care, including but not limited to:

• Advanced Care Planning

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

• Prescribe all necessary medications and treatments

• Perform comprehensive physical examinations of patients

• Develop and implement patient management policies and procedures

• Facilitate referrals to other healthcare professionals and medical facilities

• Communicate with attending physicians and/or specialists regarding patient care
