Job summary

Aviata Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays in Sarasota, FL. You will work with a multidisciplinary team to reduce weekend hospitalizations and provide comprehensive care to short- and long-term residents.

You will evaluate new-admission visits, coordinate discharge planning, and manage changes in patient condition under established protocols.

Qualifications

• Master's degree required.
• Active Florida APN license (Advanced Practice Nurse).
• DEA registration number required.
• Experience in nursing or medical fields preferred.
• Excellent written and verbal communication skills.

Nurse Practitioner (Skilled Nursing)

• As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services on Saturday and Sunday.
• You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitilizations on weekends.
• You will work with the entire facility's patient population - short and long-term residents with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians
• Evaluation of new-admission visits
• Coordination of discharge visits
• Evaluations for patients exhibiting changes in condition
• As a SNF-ist, you will provide types of care, including but not limited to:
• Advanced Care Planning
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with attending physicians and/or specialists regarding patient care
• A healthy work-life balance with flexibility.
