Job summary

Aviata is seeking a Nurse Practitioner (Skilled Nursing) to provide internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays in Florida. You will collaborate with the care team to reduce hospitalizations and manage a broad patient population, including admissions, discharges, and ongoing evaluations.

The ideal candidate holds an APN license in Florida, active DEA number, a Master’s degree, and demonstrated leadership in healthcare.

Responsibilities

• Provide internal medicine services for weekend SNF patients.
• Collaborate with interdisciplinary team and physicians to reduce weekend hospitalizations.
• Work with residents and nursing staff under medical directors and attending physicians.
• Evaluate new-admission visits.
• Coordinate discharge visits.
• Evaluate changes in condition and adjust care as needed.
• Prescribe medications and treatments under protocol.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and facilities.
• Communicate with attending physicians and specialists regarding patient care.

Nurse Practitioner (Skilled Nursing)

Major Responsibilties

• As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.

• You will collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends.

• You will work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians

• Evaluation of new-admission visits

• Coordination of discharge visits

• Evaluations for patients exhibiting changes in condition

• As a SNF-ist, you will provide types of care, including but not limited to:

• Advanced Care Planning

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

• Prescribe all necessary medications and treatments

• Perform comprehensive physical examinations of patients

• Develop and implement patient management policies and procedures

• Facilitate referrals to other healthcare professionals and medical facilities

• Communicate with attending physicians and/or specialists regarding patient care
