Carease Health is seeking an experienced Nurse Practitioner or Physician Assistant with a geriatrics or internal medicine background to perform weekend rounds and deliver preventative care to residents in skilled nursing facilities. The role combines on-site visits with remote work, supported by a dedicated scribe team for efficient documentation.

Success depends on building strong facility relationships, keeping a patient-centered approach, and collaborating with the team.

Competenze

• Active NP or PA license in the state of practice.
• 1+ year of geriatric or internal medicine experience.
• Documentation review experience and strong communication skills.

Mansioni

• Provide weekend rounds and preventative care to residents in SNFs.
