Job summary

American Senior Communities is seeking a Licensed Practical Nurse (LPN) for a weekend night position at Heritage Park. The role covers Saturday and Sunday 6p-6a, focusing on delivering high-quality geriatric care and building meaningful relationships with residents and staff.

The LPN will conduct assessments, document changes, and collaborate with nursing and facility teams to ensure comprehensive care aligned with CARE values: Compassion, Accountability, Relationships, and Excellence.

Qualifications

• Current and valid Licensed Practical Nurse license in Indiana.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in resident condition.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.

Responsibilities

• Assess and monitor resident health status and document changes.
• Provide compassionate, resident-centered nursing care during 6pm–6am shifts.
• Collaborate with care team to ensure coordinated, high-quality care.
• Promote teamwork and maintain a respectful, positive work environment.
• Guide and mentor staff as needed to maintain CARE values.
