Job summary

BrightStar Care is seeking a licensed LVN for weekend home health visits on a PRN basis. You’ll provide one-on-one skilled care in patients’ homes, administer medications, monitor conditions, and document care plans, all while working independently.

Qualifications

• Current LVN license in good standing.
• Home health experience is a plus but not required.
• Comfortable working independently in a home setting.
• Reliable and available for weekend visits.
• Strong communication and documentation skills.

Responsibilities

• Administering medications and prescribed treatments
• Providing wound care
• Monitoring patients and reporting changes in condition
• Reinforcing patient and family education
• Following established care plans and physician orders
• Communicating patient needs and concerns with the care team
• Completing accurate and timely visit documentation
• Traveling to patients’ homes for scheduled visits
