Job summary

OVERLANDPARK is seeking a qualified healthcare professional to coordinate plans of care for residents, communicate with families, and support a multi-disciplinary team. The role requires nursing or social work training, with RN/LPN licensure preferred, and CCM certification a plus.

The incumbent will manage utilization review, admissions and discharges, and ensure accurate documentation. A focus on resident safety and high-quality care is essential in a fast-paced environment.

Qualifications

  • Knowledge of Medicare/Medicaid programs and related benefits.
  • PCC knowledge and nursing workflows.
  • Ability to read and interpret technical procedures and policies.
  • Accurate documentation and record-keeping.
  • Strong problem solving and clinical reasoning.

Responsibilities

  • Communicate with residents and families about care plans and treatment protocols.
  • Attend morning huddles to coordinate with the team.
  • Answer questions regarding care, progression, and treatment options.
  • Monitor resident status and adjust plans as needs change.
  • Organize daily work based on new and ongoing resident needs.
  • Complete required documentation in a timely manner.
  • Perform utilization review to ensure appropriate, timely care.
  • Coordinate care with residents, providers, and payers.
  • Oversee admissions and discharges and coordinate referrals.
  • Maintain professional interactions with residents, families and staff.

Job description

Responsibilities
  • Communicate regularly with residents and their families about Plans of Care, PT, OT and other treatment protocols.
  • Attend and participate in morning meetings/stand‑up to facilitate communication with the team.
  • Answer residents’ questions about their care, treatment plans, illness progression and all other issues so they feel safe and secure.
  • Monitor and adjust residents' status based on changing needs and conditions.
  • Organize and prioritize daily work by assessing new, current and discharging residents’ needs in area(s) of responsibility.
  • Complete documentation as required.
  • Perform utilization review activities to provide residents appropriate, timely and cost‑effective care.
  • Coordinate care with residents, care providers, facility financial services and third‑party payers.
  • Oversee all admissions and discharge activities.
  • Coordinate referrals both to and from the facility.
  • Relate positively, effectively and appropriately with residents, families, staff and professional colleagues.
  • Maintain accurate charting and complete necessary paperwork in a timely manner.
  • Work independently and exercise sound judgment in interactions with physicians, providers, payers and family members.
  • Promote cooperation and collaboration among residents/families/caretakers, physicians, nurses and ancillary partners.
  • Exhibit excellent time‑management skills to develop organized work processes in a high‑volume environment with rapidly changing priorities.
  • Use intermediate computer skills to support work functions.
  • Maintain professional, respectful, honest interactions with residents, families, staff and partners.
Supervisory Requirements

This position has no supervisory responsibilities.

Qualifications
  • Education: Bachelor’s Degree in Nursing or Social Work.
  • License: Registered Nurse (RN) license preferred; Licensed (LVN or LPN) nurse acceptable.
  • Experience: Two (2) years clinical nursing experience preferred.
  • Knowledge of Medicare, Medicaid and related benefits.
  • PCC knowledge.
  • Language Skills: Ability to read technical procedures and comprehend policy and procedure manuals.
  • Mathematical Skills: Ability to apply fractions, percentages, ratios and proportions to practical situations.
  • Reasoning Ability: Ability to solve practical problems and interpret a variety of written, oral or diagrammatic instructions.
  • Certificates, Licenses & Registrations:
    • LVN/LPN Valid License or RN Valid License in the state where working.
    • Certificate as a certified Case Manager (CCM) – a plus.
    • Must maintain all required continuing education/licensing and be in good standing with the Department of Public Health / License and Certification Division at all times.
