Job summary

Maple Farm seeks a Licensed Practical Nurse (LPN) to provide high-quality resident care on an every-other-weekend schedule. You will administer medications, perform treatments, supervise CNAs, and help maintain safety and dignity for residents in a skilled nursing setting.

• Current and valid PA LPN license.
• Strong judgment and leadership skills.

Responsibilities

• Comply with policies, procedures, and standards.
• Follow Residents’ Rights policies.
• Maintain confidentiality of resident information.
• Follow nursing standards and facility policies.
• Communicate information between shifts to ensure continuity of care.
• Conduct daily resident rounds and report issues to nursing supervisor.
• Identify safety hazards and initiate corrective actions.
• Supervise CNAs and coach per schedules.
• Ensure regulatory compliance within the unit.
• Build relationships with residents and families to convey preferences to CNAs.
• Administer and document direct resident care, medications, and treatments.
• Notify physicians of changes in condition or errors.
• Participate in care plan development and implementation.
• Respond promptly to resident calls for assistance.
• Count narcotics and manage emergency kits.
• Provide instruction to CNAs on care procedures.
• Provide direct resident care as needed.
• Collaborate with care team and external providers.
• Participate in meal service while monitoring dietary needs.
• Order, receive, and store medications.
• Maintain resident safety and universal precautions.
• Maintain professional appearance and punctuality.
• Participate in QA activities and trainings.
• Engage residents in activities and document participation.
