Job summary

CommCare Corporation in Louisiana is seeking a Floor Nurse LPN for a weekend option at Old Jefferson Community Care Center. In this role you will ensure residents receive the highest level of care while leading on clinical tasks and collaborating with a dedicated team.

The position requires an active Louisiana LPN license and prior long-term care experience. You will evaluate resident needs, review care plans, supervise Nursing Assistants, administer medications, and liaise with physicians.

Qualifications

• Current Louisiana LPN license, active.
• Long-term care experience preferred.
• Not excluded from Medicare/Medicaid programs and maintains eligibility.

Responsibilities

• Evaluates the physical, emotional, and social needs of residents.
• Reviews residents' care plans, ensures plans are followed and documents medical records.
• Supervises care provided by Nursing Assistants on the unit, with intervention and education.
• Prepares and administers medications and treatments.
• Consults with physicians.
• Receives and records orders as required.
